Forms

Here you will find links to church releated forms to be filled out and sent to the church office. These are the computer equivalent of paper forms, only these can be filled in using your computer and emailed to the office instead of mailing or delivering in person.

Microsoft Word or Excel is required to edit these forms. If you do not have Microsoft Word or Excel, or have access to the program, you can download a free office suite called Open Office (click here). It will read and write Microsoft Office documents. Be sure that if you use this program you email the document back to us as a Microsoft document - not as an Open Office document.

Steps to Edit the forms below:

1. Click on the desired form link
2. You may be prompted to save the file to your computer
If so, save in a location easily remembered (like My Documents)
3. Edit the form and then save it to your computer
4. Using your email program, email the form back as an attachment

4. Click on the download and install link
5. Click on the computer system you are using (windows, linux, apple)
6. Follow the specific instructions to install based on your system
